Youth Ministry Registration & Consent Form

This form gives permission for your children 12-18 years to attend youth meetings and activities at and through North Edmonton Christian Fellowship.

Information collected if confidential and is used for the purpose of serving your child(ren) while in the care of North Edmonton Christian Fellowship. This form must be filled out by a parent/guardian.
