Discovering Grace Registration & Consent Form
This form gives permission for your children 4-14 years to attend DISCOVERING GRACE at and through North Edmonton Christian Fellowship.
Information collected is confidential and is used for the purpose of serving your child(ren) while in the care of North Edmonton Christian Fellowship. This form must be filled out by a parent/guardian.
Payment $25/child
Please be ready to submit payment following filling out the registration form.
PLEASE NOTE: When you SUBMIT the form, you will be re-directed to the PAYMENT page. Select “DISCOVERING GRACE” in the drop-down menu. To best allow us to connect your payment with the registration, please use the sameAdult Name on the registration page as on the payment page.
