Parental Waiver and Liability Release *

I hereby give permission for my child to be photographed and/or filmed without any reimbursement of any kind due to me. North Edmonton Christian Fellowship considers all information personal and confidential, and will only use it within the Church for administrative purposes and/or ministry, and will not release it to outside parties or organizations.

Medical Release *

In the event of an emergency and medical treatment is required, I give permission to the Church staff and/or volunteers to obtain services for treatment as deemed necessary if I cannot be contacted in a reasonable amount of time. In addition, I will not hold the staff and/or volunteers of North Edmonton Christian Fellowship liable for any loss or injury that may be sustained. I recognize there may be risks involved with the activities my child may be involved in, and thus waive the rights of lawful action against North Edmonton Christian Fellowship, their staff and/or volunteers.
